As part of the Merthyr Primary Care Cluster project, your practice team now has the benefit of an expert General Practice Support Officer. Your GPSO is available to help you with concerns or issues your GP cannot help with. Such as:
- Mental health problems
- Feeling worried or anxious
- Carer’s support
- Parenting issues
- Financial issues, such as benefits
- Housing advice and adaptations
Your General Practice Support Officer will support you with all your non-medical needs. General Practice Support Officers are specialists and have excellent knowledge of all the resources in your area, for example, mental health support, volunteering, fitness and financial support.
Accessing support from a General Practice Support Officer
General Practice Support Officers are based across all GP practices in Merthyr Tydfil and are available on weekdays. Please contact your practice to make an appointment with a GPSO.
General Practice Support Officers will receive referrals from the team within each practice.
What happens next?
After referral, your General Practice Support Officer will reach out to assess your needs. Appointments are customised in length and frequency, catering to individual needs. While some patients might need brief practical support, typically spanning one or two sessions, others may require ongoing assistance. Home visits can be arranged to address household needs, ensuring personalised support is tailored to each patient’s unique circumstances.
