Did you know that thousands of people in Wales are at risk of developing Type 2 diabetes? – but it doesn’t have to be that way.
Here in Merthyr Tydfil, the All Wales Diabetes Prevention Programme (AWDPP) is available to support people who are at risk of developing Type 2 diabetes and take positive steps towards a healthier future.
This FREE NHS programme is available to people who are registered with a GP practice in Merthyr Tydfil and have had a blood test showing they’re at risk.
What is the All Wales Diabetes Prevention Programme?
The AWDPP is a supportive programme designed to help you:
- Understand what being at risk of Type 2 diabetes means
- Learn how to make healthy changes to your eating and activity habits
- Build skills and confidence to take control of your own health
- Reduce your risk of developing Type 2 diabetes
The programme is delivered by a trained Support Worker in a relaxed and friendly way via a 30-minute one-to-one consultation, which can take place in person, over the phone, or virtually – whichever suits you best. In-person appointments are currently available at Keir Hardie and Treharris Primary Care Centre, if this is your preference.
During your consultation, the Support Worker will talk to you about your risk of developing Type 2 diabetes and what steps you can take to lower it. They may also signpost you to other support services if needed. A follow-up appointment will take place 12 months later to reassess your progress.
Is this programme for me?
You might be invited to take part if:
- You’ve had a blood test showing a HbA1c result between 42-47 mmol/mol
- You’re registered with a GP in the Merthyr Tydfil area
- You haven’t already been diagnosed with Type 2 diabetes
If you’re not sure, speak to your GP or practice nurse – they can let you know if you’re eligible and refer you.

Preventing Type 2 diabetes can help protect your health now and in the future. With the support of the programme, many people go on to lower their blood sugars, lose weight, and feel more in control of their health.
