What are Health Clusters and what do they do?
Health clusters in Wales are collaborative networks of GP practices working with a wide range of local health professionals and organisations, including pharmacists, nurses, therapists, physiotherapists, optometrists, dentists, social care workers, and partners from the third sector and voluntary organisations.
The purpose of these collaborative groups is to deliver high quality, community based services, reduce pressure on other parts of the health system, and provide care that is closer to home, more joined up, and sustainable.
For patients, the benefits include access to the right expertise and services more quickly and easily. In many cases, this may mean being seen by a professional other than a GP as the first point of contact.

Our cluster serves a local population of approximately 60,000 people, delivering health and social care designed to meet the needs of our community. By focusing on local priorities, we work together to strengthen services, improve outcomes, and make the best use of staff and facilities in our area.
Our aim is to provide care closer to home, ensuring quicker access to the most appropriate professional at the right time. For example, our physiotherapists, who have been in post for several years, have demonstrated significant benefits through direct local access and specialist expertise.
