Introduction
Your GP receptionists have received training and are now known as Care Navigators.
Care Navigators will ask a series of questions to direct you to the most appropriate healthcare professional to provide you with the most suitable and best care available for your needs.
Project Aim
Did you know that your community has a multi-disciplinary team and there are many professionals to support you with your needs? Patients often feel that they need to receive a consultation from a GP, however, there are many other professionals such as Pharmacists, Opticians, General Practice Support Officers or physiotherapists that are more suited to help with many conditions. Your care Navigators are specially trained to help identify which of these healthcare professionals is most suited to your needs, so that you are booked in and seen by the right person faster and more efficiently.
The Results
Service users can still choose to see a doctor, but the Care Navigator may suggest they see an alternative professional that can help with their condition.
By accessing the most appropriate professional for your needs, this allows you to be seen much quicker, whilst enabling others to see the GP, who need them most.
